Prominence Care Group

Prominence Care Group Ltd  provides a unique combination of personalised care, support and housing solutions for adults with learning disabilities, sensory Impairment, autism and mental health, who need care but do not need to be in an inpatient setting. We support our service users to live safely and independently, limiting the time they need to spend in restrictive settings.

About Our

Supported Living Service 

Our Supported Living provision is outcome focused with the aim of improved independence and social inclusion. We adopt a person centred approach whereby the personal needs and goals of the people we support are at the centre of everything we do. We have a robust transitional plan in place to ensure that care for our service users is seamless. Care plans are individualised and person centred. This is the heart and centre of our provision. 

All service users have separate tenancy agreements and service user care agreements. We recognise that people living with complex needs can find it challenging to live alone, to manage day to day tasks and often, experience difficulty in living independently in the community.

Our Mission 

  • A person-centred approach will be essential to deliver real change in the lives of people with mental health. 
  • Our objective is to enable people with mental health to have access to a health service designed around their individual needs, with fast and convenient care delivered to a consistently high standard and with additional support where necessary.
  • We will ensure that people with mental health, including those from minority ethnic communities, have the same right of access to mainstream health services as the rest of the population.
  • Our objective is to enable people with mental health  and their families to have greater choice and control over where and how they live.
